Create a person-centred care plan
The results of the continence assessment are used to create a person-centred care plan that includes the right toileting times, the right incontinence product type, size and absorption plus appropriate skin care products.
Ensure care plan is implemented and followed
Person-centred care is delivered through proactive toileting routines using TENA products and TENA SmartCare digital solutions - all selected to meet the resident’s individual needs. TENA also provides training and support.
Review and evaluate care outcomes
Care plan outcomes should be reviewed and evaluated regularly to identify where improvements can be made. This is done using the TENA Review and Follow-Up Report. With this information, care plans can be adapted if required to ensure quality of care.
